APPLICATION AND REQUEST FORMS

Forms for candidates and applicants intending to obtain an air carrier certificate:

  1. Planned activity declaration form;
  2. Application form for air carrier certificate.

Forms for the air carrier:

  1. In the form of a request to change the air carrier’s certificate / to grant special permits;
  2. Application form for issuing a duplicate air carrier certificate;
  3. In the form of a request for approval of an aircraft lease agreement / notification of an aircraft lease agreement;
  4. Application for obtaining / changing ALTMOC.

When the carrier changes the appointed person, for which the prior approval of the Public Transport Competence Agency must be obtained in accordance with Regulation (EU) No. 965/2012 requirements, please provide an information table on the designated person to be changed (the designated person is confirmed in the air carrier’s Flight Operations Manual):
